<p class="MsoNormal"><span style="font-family: Arial;">Lasers are ubiquitous in aesthetic medicine.<span style="mso-spacerun: yes;">&nbsp; </span>Other than Botox and fillers, no one technology is used more often.<span style="mso-spacerun: yes;">&nbsp; </span>The only problem is that there are a bewildering array of lasers available and an even more bewildering barrage of advertising to go with them.<span style="mso-spacerun: yes;">&nbsp; </span>The truth does set you free, but finding it on the Internet can be challenging.<span style="mso-spacerun: yes;">&nbsp; </span></span></p>
<p class="MsoNormal"><span style="font-family: Arial;">&nbsp;</span></p>
<p class="MsoNormal"><span style="font-family: Arial;">The word laser itself evokes images of 1960s science fiction where a giant beam is used to destroy the earth (Dr. Evil in the movie Austin Powers comes to mind).<span style="mso-spacerun: yes;">&nbsp; </span>Since the inception of lasers in medicine, there has always been some degree of optimism or even wishful thinking about them.<span style="mso-spacerun: yes;">&nbsp; </span>Until recently, the results have been somewhat disappointing.<span style="mso-spacerun: yes;">&nbsp; </span>Laser is an acronym for &ldquo;<em>light amplification by stimulated emission of radiation.&rdquo;<span style="mso-spacerun: yes;">&nbsp; </span></em></span><span style="font-family: Arial;">Light of a given wavelength is excited and then focused into a very concentrated beam.<span style="mso-spacerun: yes;">&nbsp; </span>Different wavelengths are attracted to different molecules in our bodies such as water, blood, fat, collagen, etc.<span style="mso-spacerun: yes;">&nbsp; </span>It is this interaction that bears fruit for medical applications.<span style="mso-spacerun: yes;">&nbsp; </span>In aesthetic medicine, people are interested in ways to rejuvenate or resurrect the skin.<span style="mso-spacerun: yes;">&nbsp; </span>We are all looking for ways to stop time and avoid surgery.<span style="mso-spacerun: yes;">&nbsp; </span></span></p>
<p class="MsoNormal"><span style="font-family: Arial;">&nbsp;</span></p>
<p class="MsoNormal"><span style="font-family: Arial;">The art of facial resurfacing has come a long way.<span style="mso-spacerun: yes;">&nbsp; </span>As a medical art, it began with French aestheticians who used phenol to smooth wrinkles and tighten skin.<span style="mso-spacerun: yes;">&nbsp; </span>Plastic surgeons got a hold of this technology and refined it further.<span style="mso-spacerun: yes;">&nbsp; </span>The only problem was significant downtime and significant complications such as scarring and pigment changes, things that could radically alter someone&rsquo;s appearance. </span></p>
<p class="MsoNormal"><span style="font-family: Arial;">&nbsp;</span></p>
<p class="MsoNormal"><span style="font-family: Arial;">Next came CO2 lasers which had a 10,600 nm wavelength.<span style="mso-spacerun: yes;">&nbsp; </span>CO2 is attracted to water among other things, and water is a big part of the skin.<span style="mso-spacerun: yes;">&nbsp; </span>It was, and, is a powerful device. <span style="mso-spacerun: yes;">&nbsp;</span>Unfortunately, CO2 had many of the same problems as Phenol, including significant downtime, with raw, wounded skin for weeks to months.<span style="mso-spacerun: yes;">&nbsp; </span>During the late 1990s, engineers started fiddling with fractionation, or breaking up, the actual laser beams into little spots to speed healing time.<span style="mso-spacerun: yes;">&nbsp; </span>Early devices of this type were &ldquo;non-ablative&rdquo; in that they did not damage the skin surface.<span style="mso-spacerun: yes;">&nbsp; </span>They penetrated into the deeper skin layers.<span style="mso-spacerun: yes;">&nbsp; </span>The only problem was that the clinical results were typically mediocre and required multiple treatments.</span></p>
<p class="MsoNormal"><span style="font-family: Arial;">&nbsp;</span></p>
<p class="MsoNormal"><span style="font-family: Arial;">More recently, fractionation has been applied to &ldquo;ablative&rdquo; lasers that treat both superficial and deep skin layers.<span style="mso-spacerun: yes;">&nbsp; </span>Fraxel, an Erbium laser, came first. It was then followed by fractionated CO2 devices.<span style="mso-spacerun: yes;">&nbsp; </span>Fraxel was relatively effective but was painful and often needed multiple treatments (5-6) to see an effect.<span style="mso-spacerun: yes;">&nbsp; </span>Now, CO2 devices are available that are more powerful, safely deliver more energy, and require fewer treatments.<span style="mso-spacerun: yes;">&nbsp; </span></span></p>
<p class="MsoNormal"><span style="font-family: Arial;">&nbsp;</span></p>
<p class="MsoNormal"><span style="font-family: Arial;">The best of these devices (in my opinion), and the one used at The Refine Institute is the <strong>DOT laser</strong></span><span style="font-family: Arial;">.<span style="mso-spacerun: yes;">&nbsp; </span>The actual beam is not only fractionated but has also been engineered to have an inverted dumbbell shape that delivers maximum energy to the deeper skin layers while leaving a small footprint above.<span style="mso-spacerun: yes;">&nbsp; </span>The heat in the deeper layers is critical in that heat stimulates new collagen formation (neocollagenesis).<span style="mso-spacerun: yes;">&nbsp; </span>This process leads to skin tightening and &ldquo;rejuvenation.&rdquo;<span style="mso-spacerun: yes;">&nbsp; </span>The skin surface is also addressed.<span style="mso-spacerun: yes;">&nbsp;&nbsp; </span>Brown spots, pores, and fine wrinkles are removed partly through vaporization.<span style="mso-spacerun: yes;">&nbsp; </span>Typically, 1 treatment is all that is necessary for younger patients (40s), while older patients with deeper wrinkles and more skin laxity require 2-3 treatments.<span style="mso-spacerun: yes;">&nbsp; </span>I also use this device for people with bad acne scarring.<span style="mso-spacerun: yes;">&nbsp; </span></span></p>
<p class="MsoNormal"><span style="font-family: Arial;">&nbsp;</span></p>
<p class="MsoNormal"><span style="font-family: Arial;">The DOT laser can also be safely used on the eyelid skin, creating a surrogate for surgical blepharoplasty in the right patient.<span style="mso-spacerun: yes;">&nbsp; </span>Downtime is relatively minor for most patients (5-7 days).<span style="mso-spacerun: yes;">&nbsp; </span>The first few days, the treated area becomes red with variable amounts of swelling.<span style="mso-spacerun: yes;">&nbsp; </span>The skin then will become very dry and scaly as the treatment DOTS begin to shed.<span style="mso-spacerun: yes;">&nbsp; </span>While healing, moisturizer and sunscreen are essential.</span></p>
<p class="MsoNormal"><span style="font-family: Arial;">&nbsp;</span></p>
<p class="MsoNormal"><span style="font-family: Arial;">Lasers will continue to evolve.<span style="mso-spacerun: yes;">&nbsp; </span>One key take home concept is that lasers, like fat grafting, rejuvenate the skin by stimulating it rather than cutting it out surgically.<span style="mso-spacerun: yes;">&nbsp; </span>You want to take your skin back in time or preserve it, not cut it out like a cancer.<span style="mso-spacerun: yes;">&nbsp; </span>Skin rejuvenation is a process and requires general maintenance, much like Botox and fillers.<span style="mso-spacerun: yes;">&nbsp; </span>Stimulating your skin every few years with a treatment like the DOT laser is, in my mind, preferable to waiting to the last minute and getting the facelift that so many are trying to avoid.<span style="mso-spacerun: yes;">&nbsp; </span></span></p>

<p class="MsoNormal" style="margin-bottom: 10.0pt; mso-pagination: none; mso-layout-grid-align: none; text-autospace: none;"><span style="font-size: 10.0pt; font-family: Verdana;">Fat grafting is an old technique.&nbsp; In 1926, Charles Conrad Miller described the infiltration offat through a hollow metal cannula. This technique laid the foundation for what we do as surgeons today.&nbsp;&nbsp;&nbsp; Unfortunately, subsequent attempts at fat grafting were met with mixed success.&nbsp; In 1956, Lyndon Peer conducted formal experiments looking at fat graft survival.&nbsp;&nbsp; He concluded that only about 50% of the graft survived.&nbsp; This study injected pessimism into the surgical community and delayed the development of fat grafting.&nbsp; With the advent of liposuction, interest in fat grafting was renewed due to the readily available graft material.&nbsp; Beginning in the early 1980s, Dr. Sidney Coleman and others resurrected the art of fat grafting by careful study of the techniques and conditions necessary for graft survival.&nbsp; Based on his research, many practitioners now use fact grafting as part of their surgical armamentarium, particularly for facial rejuvenation.</span></p>
<p class="MsoNormal" style="margin-bottom: 10.0pt; mso-pagination: none; mso-layout-grid-align: none; text-autospace: none;"><span style="font-size: 10.0pt; font-family: Verdana;">&nbsp;</span></p>
<p class="MsoNormal" style="margin-bottom: 10.0pt; mso-pagination: none; mso-layout-grid-align: none; text-autospace: none;"><span style="font-size: 10.0pt; font-family: Verdana;">With the passage of time, facial skin sags for two reasons. &nbsp;First, collagen and elastic fibers, which are part of the scaffolding of the skin, diminish in number.&nbsp;&nbsp; Second, there is fat atrophy which leads to facial wasting and hollowness which are indicative of facial aging.&nbsp;&nbsp; Restoring this lost fat volume thus restores a youthful appearance.&nbsp;&nbsp; Furthermore, and this is the exciting part, Dr. Coleman and others have astutely noted that placement of the fat often changed the condition of the overlying skin. The skin, for some unknown reason, looked more youthful.&nbsp; There appeared to be a resurrection factor hidden in fat.&nbsp;</span></p>
<p class="MsoNormal" style="margin-bottom: 10.0pt; mso-pagination: none; mso-layout-grid-align: none; text-autospace: none;"><span style="font-size: 10.0pt; font-family: Verdana;">&nbsp;</span></p>
<p class="MsoNormal" style="margin-bottom: 10.0pt; mso-pagination: none; mso-layout-grid-align: none; text-autospace: none;"><span style="font-size: 10.0pt; font-family: Verdana;">In the last several years, the medical community has caught a glimpse of why skin rejuvenation with fat grafting is happening;&nbsp;the involvement of&nbsp;stem cells.&nbsp;&nbsp; Many basic science researchers have discovered the presence of stem cells in transplanted fat. &nbsp;As you probably know, stem cells are a very hot topic.&nbsp; Stem cells can be used to build new organs, skin, and other body parts for the sake of reconstruction.&nbsp; Now it appears that stem cells carry with them a potential ability to rejuvenate the skin. The specific pathways for this process have yet to be fully delineated. This is a very exciting finding and promises to take aesthetic medicine, particularly that involving the face, into the 21st century. In my vision, the 21st century will involve fewer scalpels. Rather, rejuvenation will involve manipulation at the cellular level to coax the body into a more youthful state and appearance. This is already being done to some degree with lasers and now fat grafting is following suit.&nbsp; This is where things are headed.&nbsp; Stay tuned.&nbsp;</span></p>

Call 704.909.4800 for more information OR <a href="/Patient-Information/Online-Consultation.php">click here to register</a> and a staff member will be in touch.</p>]]></content></entry><entry><title><![CDATA[SMARTLIPO AND OTHER LASERS IN LIPOSUCTION]]></title><id>http://www.refineinstitute.com/News/ID9/SMARTLIPO-AND-OTHER-LASERS-IN-LIPOSUCTION</id><updated>2009-01-26T00:00:00-05:00</updated><link>http://www.refineinstitute.com/News/ID9/SMARTLIPO-AND-OTHER-LASERS-IN-LIPOSUCTION</link><summary><![CDATA[SMARTLIPO AND OTHER LASERS IN LIPOSUCTION]]></summary><content type="html"><![CDATA[<p>Traditional liposuction is one of the more traumatic procedures we perform as plastic surgeons.&nbsp; The simple to and fro motion of a blunt liposuction cannula generates significant tissue trauma, swelling, and prolonged recovery.&nbsp; Due to this fact, a lot of energy has been spent searching for kinder and gentler alternatives.&nbsp; One potential alterative promoted in recent years involves lasers.&nbsp; <br /><br />The word LASER is an acronym for Light Amplification by Stimulated Emission of Radiation.&nbsp; Lasers have many important applications in medicine, including the targeting of fat cells.&nbsp; Beginning in the early 1990s, Deka, an Italian laser company, developed a 1064 nm Nd:YAG laser that targeted and ruptured fat cells.&nbsp; Today, there are three FDA-approved laser-assisted liposuction systems available in the U.S., including SMARTLIPO (Cynosure), COOLLIPO (CoolTouch), and PROLIPO (Sctiton).&nbsp;&nbsp; <br /><br />The real question is this: Is laser-assisted liposuction better than traditional liposuction?&nbsp; Information on the internet regarding lasers and liposuction is confusing for patients, and it is hard to carve through both the hype and cynicism and get to the truth. <br /><br />Proponents of laser-assisted liposuction are often non-Plastic Surgeons who recently joined the field of body contouring after practicing other forms of medicine, including ENT, Dermatology, ER Medicine, Family Practice, and Obstetrics and Gynecology.&nbsp; These practitioners often oversell the benefits of this technology since it is often the only arrow in their body contouring quiver.&nbsp; Opponents of laser-assisted liposuction are typically Plastic Surgeons who have not used the technology and take a rather blunt and defensive posture.&nbsp; As is usually the case, the truth&nbsp; lies somewhere in the middle.&nbsp; Here is some background to help you decide for yourself:</p>
<p><strong>HISTORY</strong><br />As mentioned previously Laser-assisted liposuction was developed in Italy by DEKA in the early 1990s.&nbsp; The first clinical description of laser-assisted liposuction was in 1992 by Apfelberg.1&nbsp; In 1996, a multi-center FDA trial comparing standard liposuction to laser-assisted liposuction was performed.&nbsp; Laser-assisted liposuction possessed a shorter recovery time and less bleeding, but the clinical outcomes were equivalent and the sponsoring company, Haraeus Lasersonics, abandoned the technology.2&nbsp; This study has played a significant role in shaping plastic surgery opinions about laser-based liposuction.&nbsp; One critical point to make regarding this study is that it involved a laser whose tip was not in direct contact with the fat.&nbsp; It was rather housed within a large cannula (between 4-6 mm), thus making it potentially much less effective than what is on the market today.&nbsp;&nbsp; This is particularly true when it comes to treating the skin.</p>
<p>In 2005, Badin studied a 1064 nm laser whose fiber extended past the distal end of a 1 mm cannula, thus putting the laser in direct contact with adipose tissue.3 Badin found laser-assisted lipolysis was less traumatic than liposuction, due in part to the use of this much smaller cannula.3 In addition, he also noticed the Nd:YAG system improved skin retraction.3&nbsp; Since then, other studies have followed, demonstrating coagulation of blood vessels, and coagulative necrosis of collagen, which leads to new collagen formation and skin tightening.4<br /><br /><strong>HOW IT WORKS</strong>&nbsp; <br />Lasers are focused light energy.&nbsp; Depending on the wavelength of that light energy, it will be attracted to and absorbed by specific substances in your body.&nbsp; For example, the Nd:YAG laser, the one used in SmartLipoTM, has a wavelength of 1064 nm.&nbsp; This wavelength is attracted to water.&nbsp; Fat cells and skin contain water.&nbsp; When fat cells contact this laser, they absorb the energy, swell, and then rupture, draining their contents.&nbsp; Once this occurs, an oily residue is suctioned away much like traditional liposuction.&nbsp;</p>
<p>One of the key reasons that laser-assisted liposuction is gentler is that the energy is delivered through a very fine cannula (1 mm) which houses an even smaller laser fiber (around 300 microns in diameter).&nbsp; This is in contrast to traditional liposuction, which uses mechanical energy generated by shoving a much larger cannula (3-6 mm) to and fro.&nbsp; Due to its inherent gentleness, laser-assisted liposuction is easier to perform without general anesthesia.&nbsp; Smaller cannulas and non-mechanical disruption of the fat cells means less trauma to the patient and a faster recovery.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; <br /><br /><strong>FDA CLEARANCE?&nbsp; </strong><br />There is a misconception that laser-assisted liposuction is a fringe, experimental technology that is not FDA approved for clinical use.&nbsp; This is incorrect.&nbsp; In the case of SmartLipoTM, a system I use at The Refine Institute, formal FDA clearance was obtained in November of 2006.&nbsp; Prior to that, thousands of patients had been successfully treated in Europe, Asia, and South America. Since its approval by the FDA, SmartLipoTM has been shown to be consistently safe and effective when used by a trained Plastic surgeon.&nbsp; <br /><br /><strong>CLINICAL BENEFITS?</strong><br />As has been shown in several studies and in general clinical experience, laser-assisted liposuction results in less swelling, bruising, and downtime.&nbsp; In addition, the laser can heat the undersurface of the skin, causing skin contraction or shrinkage.&nbsp; This means you can use the laser in areas of loose skin where the use of traditional liposuction would not be an option.&nbsp; The clinical results clearly support this reality.<br /><br /><strong>SMARTLIPO'S&nbsp; ROLE IN BODY CONTOURING?</strong><br />One of the most common questions patients ask is whether Smartlipo is a replacement for traditional liposuction.&nbsp; The answer is specific to each patient and their needs.&nbsp;&nbsp; Lasers can tighten skin and lessen trauma and downtime, but they are not currently an effective tool for removing large volumes of fat.&nbsp; This is where traditional liposuction, particularly power-assisted liposuction, remains the gold standard.&nbsp; Many plastic surgeons who have embraced laser-assisted liposuction use it in conjunction with traditional liposuction.&nbsp; It is this hybrid approach that has the potential to provide optimal care and better patient outcomes.&nbsp; <br /><br />As a final clinical and ethical caveat, I believe it is extremely important to apply this technology with realistic expectations in mind and not to oversell its capabilities to patients.&nbsp; I concur with Dr. Alberto Goldman, a Brazilian plastic surgeon and forefather of laser-assisted liposuction,&nbsp; who made the following statement:<br /><br />"SmartLipo is an advance, not a miracle."</p>]]></content></entry><entry><title><![CDATA[THE REFINE INSTITUTE INTRODUCES FIRST VECTRA 3D IMAGING SYSTEM IN THE CAROLINAS]]></title><id>http://www.refineinstitute.com/News/ID8/THE-REFINE-INSTITUTE-INTRODUCES-FIRST-VECTRA-3D-IMAGING-SYSTEM-IN-THE-CAROLINAS</id><updated>2009-01-15T00:00:00-05:00</updated><link>http://www.refineinstitute.com/News/ID8/THE-REFINE-INSTITUTE-INTRODUCES-FIRST-VECTRA-3D-IMAGING-SYSTEM-IN-THE-CAROLINAS</link><summary><![CDATA[Vectra's 3D Sculptor Tools Offer Patients Preview Of Post-Surgical Results Using The Most Advanced 3D Imaging Technology Available]]></summary><content type="html"><![CDATA[<p>CHARLOTTE, N.C. <p>The system, designed by Canfield Scientific, Inc. uses special cameras with multiple lenses to capture a 360 degree image of the patient in less than two milliseconds.&nbsp; The image is then loaded into the Vectra 3D Sculptor Tool software and used during the patient&rsquo;s consultation.&nbsp; The software allows the doctor to show the patient multiple views of the body, simulated outcomes and even side-by-side before and after pictures---all before surgery is performed.&nbsp;</p>
<p>Dr. Andrew Gear, who leads the new practice, said this technological breakthrough will make it easier for patients to review their options.</p>
